Primary Care Pharmacist - North East

Job Description

Our Client is responsible for commissioning (buying) and providing health care services for the city's 271,000 residents.

Making sure that people can access services such as GPs, district nursing, hospital treatment, mental health services, screening programmes, dentists, pharmacists and opticians.

T he role will include but not be restricted to the following:

To support the delivery of local and national prescribing policy. To implement the PCT prescribing and medicines management strategy and Action Plan in GP practices.

To promote the safe, rational, high quality and cost effective use of medicines within allocated GP surgeries by working closely with medical staff, healthcare managers and the primary healthcare team. To encourage cost effective prescribing by monitoring, analysing prescribing data and making recommendations for a change in practice to others.

Experienced Required

To set up clinics e.g. for medication review, chronic disease management, medication monitoring and health promotion and to provide a personal professional/clinical contribution as a member of the multidisciplinary practice team.

To provide specialist pharmaceutical advice to other members of the practice team and to patient's e.g. therapeutic use of drugs, medicines supply issues, GP computer system, use of unlicensed medicines etc.

To support the achievement of practice based clinical governance through peer review, holding practice meetings, audit, education/training of other members of the healthcare team (e.g. GPs, Practice Nurses) and providing support to less experienced staff (e.g. pre-registration pharmacists, pharmacy students).

The post holder may work as a supplementary/ independent prescriber and following discussion with the prescribing advisers as to remit and strategic direction of the role follow individual care plans agreed with the GP and patient.

To review patients' notes and prescriptions and to provide specialised advice to patients, medical staff and other healthcare professionals, in order to ensure the appropriateness of medicines use.
